We may also ask you to provide health information including your medical history, results of medical tests, health concerns and details of your health care professionals.

We generally collect your personal information in person, in writing (for example, hen you complete our New Patient Form), via our website, or by telephone, email or video conference call over the internet.

We may also, with your consent, collect information from third parties such as your medical practitioner or mental health professional.

You can choose not to provide us with the personal information we request, however this may result in us not being able to provide you with the services you require.

However, we are not responsible for any privacy or data security breach by or affecting our service provider.

We will take measures to destroy and de-identify personal information that is no longer required for the Services or any lawful purpose (subject to any legal requirements imposed on us, for instance, in relation to record-keeping).

We process payments using EFTPOS and online technologies. All transactions processed by us meet industry security standards to ensure payment details are protected.

Any access to, or information you provide on, third party websites is at your own risk and we do not accept responsibility or liability that may arise from you accessing any linked website.
